Challenge

Tracking community health indicators is essential to improving evidence-based decision making in government, non-profit organizations, and businesses. Patient surveys are often the best method to gauge the effectiveness of health-related social programs. They facilitate an objective process of answering questions related to program effects, implementation, and purposes. Such evaluation provides detailed and objective information for enabling improvements in program performance.

Solution

Vizable Insights teamed up with Washington Nonprofits to develop a sample report of survey results. For this sample dashboard, we generated mock data, loosely based on Patient Activation Measure (PAM) survey conducted by Insignia Health, Oregon based company supporting academic health research.

Result

Our two-part dashboard provides an overview of the progress of each care program, shows progress broken down by patient ethnicity, age group, gender, and insurance type, as well as changes in patient attitudes towards their health care. The second part focuses on the demographic landscape of survey participants.
